Rebate applies only to the Azden products listed in this post. Products may be purchased online or in store. To qualify, print and fill out the NAB 2015 Rebate Program Form. Mail in a completed copy of the rebate form and a copy of your sales receipt to the address listed below. Your sales receipt must be dated between 4/1/15 and 4/30/15. Mailing must be postmarked by 5/31/15.
